Risks are not managed properly with out of date assessments and unsafe moving techniques observed.

Risk assessments did not provide accurate and up to date information relating to each person... We observed the staff transfer a person to a chair using an under the arms technique, this involved grabbing the person’s trousers.

Recruitment checks are incomplete with gaps in records and missing references for new staff.

Safe recruitment procedures were not always followed... gaps in staff’s previous employment were not always evidenced... One staff member worked with only one reference when two should have been obtained.

Not all staff have completed their mandatory training despite some improvements.

Although training compliance had improved, not all staff had completed their training... One staff member did not have a good understanding of Deprivation of Liberty Safeguards (DoLS)
