People were not consistently protected from the risk of harm and abuse. Staff did not always identify allegations of abuse or make safeguarding referrals. Medicines were not always administered safely.

Staff told us they did not feel listened to, morale is low, and there is no point raising concerns as nothing changes.

There remained a heavy reliance on agency staff who were not all familiar with people’s needs and high turnover of staff.

Records showed medicines training had expired for some staff and a refresher was not undertaken in line with the provider’s policy.

The service was without a registered manager at the time of the assessment. Staff described a lack of leadership and said they felt unsupported.

Opportunities were limited for social engagement and activities to provide stimulation and enrichment in people’s lives. Staff said this was because people were reluctant to engage.
