Managers do not have strong checks in place to spot and fix issues like risks, accidents, and training gaps.

Robust quality assurance systems were not in place to enable the provider to monitor the service... The provider's audits had not identified issues we found on inspection including with accidents and incidents, risks to people, care records, staff training and competencies.

Risk details for things like skin sores from beds or equipment checks are often missing, and some incidents like missing medicines are not reported properly.

Risks to people were not always documented... Information about specialist mattresses and the settings people should use these at was not recorded... when a high-risk medicine was found missing, this was not reported to the police, health professionals or safeguarding.

Staff training is not kept up to date as it should be, including checks on skills for giving medicines.

Staff training had not always been updated in-line with the frequency identified by the provider... Supervisors responsible for administering medicines had not always had their competency... assessed.
