some people’s needs were not met in respect of management of their skin integrity, continence... one had faeces on the toilet seat, and the other had faeces on the sink... soiled underwear left in their bathroom

Management raises hopes with a new manager but ignores staff concerns on staffing and other issues due to weak governance.

some staff said they had raised concerns to management in relation to staffing levels, food and activities... but there had been no changes made... did not have robust governance systems
