Training is patchy with gaps in basics like medication, fire safety and mental capacity, and some staff have had none for years.

The provider failed to ensure staff had received training to complete their role such as medicine administration, MCA, and fire evacuation training. One staff member said, "I haven't done any (training) since I started 2 years ago".

Risks around fire and accidents are not always checked or audited properly, so problems can go unnoticed.

Not all risks were assessed and monitored. For example, the required health and safety check certificates were not always in place and up to date. A fire risk assessment was in place but had not been reviewed within the appropriate time frame.

No activities coordinator in place yet, so the role lacks a proper programme of varied events.

The registered manager was in the process of recruiting an activities coordinator and we observed staff engaging with people during the inspection.
