Medication records miss signatures and medicines run out sometimes, putting people at risk.

missing signatures on medicine administration records (MARs) for medicines such as insulin... people had run out of these medicines.

Checks on quality and safety are weak, so issues like medicine problems keep happening.

Internal quality assurance processes were not always effective in monitoring the service... medicine audits had been completed in January 2023 and identified people had run out of medicines, however we found in June 2023 people continued to run out of their medicines.

Training had gaps until recently, with specialist courses only now being booked.
