Care isn't always safe because risks like falls, malnutrition, pressure damage, and infection control aren't handled properly every time.

Risks to people were identified, however management plans were not always in place to mitigate risks... some practices around infection control needed to improve.

Lots of agency staff, especially at night, means poor continuity and residents don't get to know the team.

Agency staff have been used a great deal, both during the day and night, so very little continuity.

No permanent manager in place, just an interim, and oversight misses big issues like poor record-keeping.

The registered manager had recently left, and an interim manager was managing the service... Records were not suitably maintained.
