Staff numbers sometimes drop below what's needed on shifts, and recruitment skips full work history checks.

Staffing levels were lower than required on some shifts... the service had not obtained a full employment history for the staff whose recruitment files we reviewed.

Medication guidance is sometimes missing so staff might not know when to give as-needed meds, and one safeguarding referral got missed.

Information to support staff to safely administer 'when required' medicines was not always available... we found one example where necessary referrals had not been made.
