"Positive culture, good training and activities, but unsafe medicines management and audits missing key issues hold it back."
Medicines were not always managed safely... the medicine count did not match the stock... Staff did not record the exact position of where the patch was applied... breach of Regulation 12
People, relatives and staff told us there was a positive culture at the service. We observed kind caring interactions... 'We are a family.' A staff member told us, 'Generally very good management and supportive.'
There were sufficient numbers of staff deployed to meet people's needs and keep people safe. Some staff told us that staffing levels could be low at times... the provider used agency staff when needed
Per 2018 comprehensive inspection: Staff had received face to face training in a range of subjects in order to perform their jobs safely... New staff received an induction... Staff had individual supervision meetings
Staff and the registered manager completed a wide range of checks and audits on the service; however, these failed to identify and address the issues we highlighted during inspection.
Per 2018 comprehensive inspection: There was an activities coordinator who was present... organised small group activities such quizzes, artwork and reminiscence... gardening... entertainers visited the service
Medication errors like stock numbers not matching records and no notes on where pain patches were applied make it unsafe.
We completed a reconciliation of medicines and found that the medicine count did not match the stock on the electronic MAR. Staff did not record the exact position of where the patch was applied
Audits and checks missed problems with medicines and care plans, so leaders are not spotting issues in time.
