Not all risks to people's safety and wellbeing were identified, assessed, and recorded... Fluid intake records viewed showed people's fluid targets were not always met... records demonstrated this [repositioning] was not happening as stated.

Sometimes there are enough staff and at other times not. I have to wait a bit at busy times... Around this time, they could do with a few more staff. I sometimes have to wait for my incontinence pads to be changed.
Risk records have gaps on things like fluid intake, repositioning, and catheter care, so staff might not have full info.
