People did not always have medicines managed safely. Medicines were not always administered when required and this meant people were exposed to increased risk of harm.
When we spoke with staff they did not feel concerns raised over the safety and risks of people at Bethany Lodge were listened to. There was a disconnect between the thought and operations of the provider and manager and what in reality staff felt as concerns.
During the visit on 12 November 2025 we found a person within the service being supporting by an agency member of staff who we could not be assured had the appropriate level of skills and knowledge to support them safely... working alone to support the person.
The risk assessment for this person identified staff required training in epilepsy and specific training to manage their anxious and distressed support needs. The agency member of staff did not have the training or experience required.
There has not been a registered manager in post since 3 June 2024, and the lack of transparency in subsequent managers reporting incidents and taking actions to protect people and staff has led to a culture which is not open and transparent.
One person’s daytime routine was limited to trips out in the car and to McDonalds with no other structure or opportunity. Other people’s care did not reflect a focus on growth or enrichment of experiences.
