Quality checks missed problems with risk plans, medication records, and call bell response times.

The provider had a range of audits and checks... However, some of these were not robust enough to identify the issues we found during this inspection in relation to risk mitigation plans, medicines management, and call bells response time records.

Staffing is enough to meet needs but staff say they work hard.

One person said, “There are enough staff, even though they [staff] do work hard.” Staff told us they felt there were enough staff to meet people’s needs safely.

Medication records had errors like missing signatures on changes, one missing as-needed protocol, and incomplete paperwork for hidden medicines.

MAR charts had duplicate medicines that had been crossed out without a second authorised signature... We only found 1 record where a PRN protocol was not in place... we did not see any authorisation documents signed by the GP, pharmacist and Next of Kin.
