Systems to monitor people’s risks were not always effective. Staff recorded incidents, including near-misses and physical violence towards staff. However, there was no system in place to identify them as incidents or to review them on a regular basis.
Staff told us they enjoyed working at the service and felt well supported. Comments included 'The environment is positive, and the team is supportive', 'one of the best places I have worked'.
