Bosses lack proper oversight and checks, so care problems like medication errors keep happening.

The provider continued to have a lack of governance arrangements in place to recognise and drive improvement to ensure people were receiving a safe and effective service.

Medication errors happened, like wrong eye drop dose for months and expired drops used.

One person had not had an eye drop administered correctly for a period of over four months. The period the eye drops had been open and in use exceeded the time frame recommended by the manufacture.

Staff training has gaps and is not kept up to date, like old fire safety training.

Per 2019 comprehensive inspection: Staff had not had access to regular training updates to refresh their knowledge in key areas. Some gaps were evident on the training matrix for example the night staff previously attended fire training during 2017.
