Some risks like bedroom steps and garden railings aren't fully recorded in assessments yet.

Records did not show how the risk of falling down the steps... had been assessed. The risk assessment for people potentially climbing over the railing... was not recorded.

Big staff turnover with lots of new starters, and some recruitment checks were missing and fixed during the inspection.

There had been significant staff changes since the last inspection... We looked at recruitment files for previously recruited staff and identified further information was needed to ensure people were suitable.

Only basic variety of activities like quizzes and trips, no dedicated coordinator or special programmes.

Social events were now delivered by a skilled staff member... including visiting musicians, quizzes, trips to the pub and cafes.
