Managers do not understand or fix repeated problems like poor records and audits from past inspections.

Leaders were unable to tell us what was cited in the previous warning notice, therefore, could not tell us what and where improvements had been made.

Skin creams are not always applied or recorded properly, with no risk checks for storage.

No safe topical creams management procedures in place. This meant we could not be assured people had their topical creams applied as prescribed.

No activities or engagement, leaving residents in bed too long with little interaction.

Large number of people residing in bed during our onsite visit... relatives shared concerns their loved ones spent too long in bed with little interaction from staff.
