The service had failed to operate effective systems to identify allegations of abuse, including the risk people may experience unnecessary or disproportionate restraint. This was a breach of Regulation 13.
Staff told us they felt comfortable raising concerns with the service management. Ward staff worked well together and with external agencies involved in care delivery.
The service had enough nursing staff. Staff told us they felt there were enough staff to meet people’s needs because some people required 1 to 1 support.
Staff completed mandatory training in essential skills including basic life support. Permanent staff had completed training in mental health awareness, supporting people with learning disabilities and autism and positive behaviour support.
Governance processes and systems were not effective and did not always ensure areas of improvement were identified and rectified in good time.
An activity coordinator had continued to offer some activities including escorted trips to the supermarket. Another completed weekly arts and crafts, facilitated by an external visitor.
Staff used physical restraints without training or proper risk plans, and unexplained injuries were not always reported to safeguarding teams.
Staff did not receive training in the use of restraint including any type of physical holds, however, we found examples of staff using holds when providing care and treatment.
Managers missed big problems like safety risks and poor staff checks because oversight and audits were not strong enough.
We identified 5 breaches of regulations that had not been identified by the provider’s own quality assurance processes.
Recruitment records for some agency and new staff were incomplete with missing references and checks.
Staff recruitment processes did not consistently promote safety. For example, 2 profiles for agency nurse staff members did not include all information required, such as evidence of continuous employment.
