We reviewed the previous 4 weeks staffing rotas. The service used a minimum of 3 staff but did not use a dependency tool... mixed feedback from staff members regarding staffing levels
The training records and competency assessments were a simple tally... it was difficult to ascertain if there were any gaps or overdue training as the provider had not maintained an effective system
Managers were not always visible and available within the service... continued lack of oversight which placed people at risk from harm
