What is the 20 minute break rule?

During the shift: They cannot make you take your break at the very start or very end of the shift

Care homes are allowed to cancel your break if there is a safety risk like if you are the only one on the floor. BUT the law says they must give you that time back later.

Do I have a right to smoke breaks?
There is no legal right to a smoke break. If you don't smoke your employer cannot force you to work while smokers go outside. This is a common grievance but it is a policy issue not a legal one.
